The ABCs of infant leukemia.

p i ( l d d R d I t e A lthough seemingly rare, leukemia is the second most common cancer in children less than 1 year of age. This represents 5 to 10% of hildhood leukemias, with an incidence of 44 per illion “infants” or approximately 200 infants diagosed with leukemia in the United States each year. or unclear reasons, this incidence appears to be ising over the past 20 years. Infant leukemias nclude subsets of both acute lymphoblastic and cute myeloblastic leukemias (ALL and AML) with nique clinical and biologic features, including ixed lineage leukemia (MLL) rearrangements. The istorical definition of “infant” leukemia is based on he observation that children less than 1 year of age espond poorly to standard therapies for older chilren with leukemia. In recent years, we have gained n appreciation for the unique biologic nature of nfant leukemias and struggled with the poor outome of our youngest patients with ALL, as well as ith the public health implications of emerging nvironmental and dietary risk factors. In the past 2 ears, four major trials have reported improved utcomes in infant ALL, bringing new hope and reater understanding. These studies will encourage ew research opportunities and intense discussion. n this review, we hope to introduce you to the urrent knowledge and discussions in this field. Leukemia can present at anytime, beginning in utero. etal leukemia has been detected by umbilical blood ampling as early as 33 weeks of gestation when ltrasound may detect polyhydramnios, hydrops, nd/or fetal hepatosplenomegaly.
